A Digital Technique of a Bone Reduction for Maxillary Full-Arch Fixed Dental Prosthesis

This article describes a digital technique that allows for precise bone reduction while simultaneously placing implants and interim prostheses.

Maxillary full-arch implant-supported fixed dental prostheses (ISFDP) present challenges in achieving optimal esthetic outcomes, particularly in patients with excessive gingival display. Bone reduction is often necessary to create prosthetic space and hide the prosthesis-tissue junction (PTJ). Bone reduction requires careful planning to avoid unfavorable outcomes.

The technique provides a digital workflow to precisely design bone reduction, implant placement, and facially driven ISFDP on a virtual patient with the verified maximum smile line. The orientation of the head plays a vital role in extrapolating the bone reduction line. The validation of both the head inclination and the positioning of the maximal smile line is undertaken within the software framework using a 2D image prior to the design phase.

Using this technique, clinicians can perform surgical procedures in a systematic manner without compromising the functional or esthetic outcomes.
