Clinical Outcomes of Implant-Supported Monolithic Zirconia Crowns

JOP_8-10-22

Now online in the Journal of Prosthodontics, a systematic review co-authored by ACP members, Wanjin Kim, DMD, Xiao Chun Li, DMD, and Avinash S. Bidra, BDS, MS, FACP.

Monolithic zirconia was initially considered unfavorable as a restorative material due to the highly opaque, white appearance of early commercial products. However, newer developments in manufacturing protocols have led to increased acceptance of monolithic zirconia as a restorative material with acceptable esthetics.

The aim of this systematic review was to examine current clinical evidence regarding the short-term and medium-term clinical survival of implant-supported monolithic zirconia single crowns and fixed partial dentures.

An electronic search of English-language literature was performed in PubMed, Scopus, and CENTRAL databases for researched published between Jan 1, 2001 and Sept 17, 2021. Qualitative and quantitative data were extracted from 14 full-text articles. Of these, 3 were designed as randomized controlled trials, 5 were prospective studies, and 6 were retrospective studies.

Of the 644 implant-supported monolithic zirconia restorations computed in this systematic review, there was only 1 reported failure of the monolithic zirconia restorative material over a follow-up period of up to 5 years, for a cumulative survival rate of 99.84%. Failure in this review was defined as a fracture or compromise of the zirconia ceramic restoration that required a permanent removal or remake of the restoration.

The results of the review show that implant-supported monolithic zirconia single crowns and fixed partial dentures have excellent short-term (<5 years) survival rates, but the evidence for medium-term survival (>5 years) and beyond is lacking. Studies with longer follow-up periods are needed to validate the continued reliability of implant-supported monolithic restorations in the long-term.
